New Clearer Applications

 

New Application Form 2.1

 

When we started we were told one of the biggest problems would be to find an application form that all our providers would accept. We decided that the best way forward was to get the panel members to design the form themselves. That first design has been our form up to now.

 

In the last few months, the ABI has come up with a number of guidance notes for application forms and we have moved to an imaged workflow system. We were also aware that we had a very functional but not very attractive design. Many of our introducers have offered advice and this has also been vectored into our thoughts.

 

At 12 pages, Version 2.1 is much shorter than its predecessor and is much easier to fill in. Both the client and the introducer's details are on the front page making communication far easier. There is also much more room to put answers and the comments page is now at the back. A new area has been created for bank details which will allow us to speed up going on risk.

 

We have also completed a much needed facelift on the all the Key Questionnaires (KQ's). In particular we have introduced a KQ1 for those who have hazardous occupations and have split the Hypertension and Chest pain KQ into 2 different KQ's. This should avoid a regular confusion in which the client missed the second element.

 

All the new forms also have the return address strategically placed so that they can be posted in a window envelope

 

The on line form is currently facing a similar face lift and will be in the new style delivery we hope will in the next two weeks. A new development already in detailed planning will be a seamless application process in which the completion of our on line forms will be the only form required.
